Client must first complete a Suboxone pre-treatment form found on their website and submit it to the clinic to be considered for the program. They will not accept this information over the phone. Suboxone program coordinator then will contact patient for further details and appointment availability. Poly-substance use is accepted on a case-by-case basis. Accepts adolescents and pregnant women on a case by case basis.
